Abstract
This study explores the application and value of digital healthcare platforms in chronic disease management, focusing on their roles in enhancing medical quality, optimizing management efficiency, and improving patient experience. By reviewing the development status and functional characteristics of digital healthcare platforms, the study finds significant advantages in health data collection, intelligent monitoring, personalized intervention, and patient–provider interaction. The findings indicate that digital healthcare platforms facilitate the transition from passive treatment to proactive health management, enhancing patients’ self-management capabilities and optimizing the use of medical resources. It is anticipated that digital healthcare platforms will play a critical role in future chronic disease prevention and the construction of smart healthcare systems.
